We’re currently seeking a Clinical Manager to join our Home Health leadership team in Farmington Hills, MI . If you’re an experienced RN looking to take the next step—or already in a leadership role and ready for a company that truly supports you—we’d love to meet you.

What You’ll Do:

As Clinical Manager, you’ll lead our clinical team with heart and precision.

• Overseeing the coordination and delivery of skilled home health services.
• Reviewing clinical documentation and plans of care.
• Leading case conference and interdisciplinary team (IDT) meetings.
• Ensuring compliance with standards and promoting best practices.
• Supporting and mentoring field clinicians.
• Building strong relationships with referral partners, patients, and caregivers.

What You Bring:

• Graduate of an accredited School of Nursing (BSN preferred).
• Active RN license in the state of Michigan (or compact license).
• Minimum 2 years RN experience, with 1+ year in leadership or strong desire to grow into a leadership role.
• Home health or hospice experience is highly preferred.
• Strong communication, organization, and team-building skills.
• Comfortable with in-office leadership expectations (4 days/week).
