Cedars-Sinai's Emergency Department (ED), with 60 patient beds, is a Level I Trauma Center, Level II Pediatric Trauma Center, Level III Geriatric Care ED, STEMI Receiving Center, Comprehensive Stroke Center and Paramedic Base Station. The ED is open 24 hours a day, seven days a week available to all people in need. **Learn more about Cedars-Sinai's ED at:** ** As an ED Care Coordinator you will work in collaboration with ED Leadership and the clinical care team as the liaison to assist identified patients 65 and older in optimally navigating the health system, delivering health education, and serving as a patient advocate thereby encouraging the patient healthcare partnership. **Primary Duties and Responsibilities:** + Provide a positive link between the ED, patient, their support group, other members of the ED Team and community supports. + Promote patient satisfaction and happiness by ensuring the well-being of patients by attempting to fulfill their needs, while reducing the stress on the patient. + Demonstrate Cedars-Sinai's core values of respect, compassion and commitment to excellence in patient care through teamwork and collaboration. + Patient advocacy through engagement, outreach, and providing linkage to resources. + Educate and connect patient to medical home, facilitate post-discharge appointments, and medication or other at-home needs through social service providers. + Assist patients and families with navigating community programs and services. + Help patients access their health insurance, meet their social services needs, and follow-up on appointments. + Work with area primary care and psychiatric clinics and providers to link patients with medical or psychiatric needs. + Work with internal and external team members and organizations to ensure a coordinated outreach approach. + Develop and maintain positive productive working relationships with health care providers and community-based services (e.g. mental health and substance abuse programs, community-based case management, and other support services). + Maintain program records, reports and data. + Follow up with patients to ensure they received the care and services recommended by the ED. + Attend meetings as required and participate on committees as directed. + Meet regularly with Emergency Department Co-Chair(s) Position is remote working three 8-Hour shifts per week. **Qualifications** **Educational Requirements:** + A High School Diploma or GED is required. + Bachelor's degree preferred. **Experience:** + Minimum one year prior experience working in health care field, engaging with patients, experience in care coordination/case management, medical office, community outreach or customer service. + Basic knowledge of community resources in Los Angeles and familiarization with community-based health care delivery. + Excellent interpersonal and collaborate skills. + Ability to speak, read and write English fluently; bilingual (Spanish/English) preferred. + Detail oriented and organized. + Ability to carry out written and oral instructions. + Ability to exercise judgment in the application of professional services. + Proficient in computer navigation skills, Microsoft excel, and online researching. **Req ID** : 11334 **Working Title** : Care Coordinator - Emergency Department - Part-time (Remote) - 8Hour Days **Department** : CSMC 7010003 Emergency Dept Support **Business Entity** : Cedars-Sinai Medical Center **Job Category** : Patient Services **Job Specialty** : Patient Relations **Overtime Status** : NONEXEMPT **Primary Shift** : Day **Shift Duration** : 8 hour **Base Pay** : $25.06 - $38.84 Cedars-Sinai is an EEO employer.
