Who We Are
Ryse Health is on a mission to create a world that is free of the burden of chronic disease. In order to do that, we have designed an innovative health care model for families and individuals living with type 2 diabetes. This model combines best-in-class therapies and technology with day-to-day support, coaching, and analytics to help patients to achieve extraordinary outcomes.
Here's what you'll be doing
You'll be responsible for helping patients overcome psychological barriers to their cardiometabolic health, including:
Assessing social determinants of health needs and developing a treatment plan
Providing personalized coaching, guidance, and encouragement to help individuals set achievable goals and develop healthier habits
Collaborating with a multidisciplinary team to create tailored strategies that promote resilience, self-awareness, and overall wellness
Creating and maintaining documentation
Building rapport and maintaining positive relationships with patients and their families
What you need to be successful
Over 2 years of experience as health or life skills coach with a strong focus on overall mental health and well-being
Excellent communication skills, empathy, and the ability to inspire positive change through motivational interviewing and other tools
Demonstrated ability to partner with other clinicians in a multi-disciplinary care environment
Passion for the impact of your work on the broader world, and excitement to help reduce the burden of chronic disease
Master's-level coursework and/or experience supporting patients navigating chronic physical disease preferred but not required
